Hi Sandy

Isnt it a nightmare trying to get this!! Im on long term sick from work and am making a claim for it just now. The SSP1 was a form my employers gave me from my pay department and then you forward this on with the sick certificates to the relevant office who are dealing with your claim. Although having said this when I mentioned this SSP1 form to the person dealing with my benefit he said they dont tend to us this form any more??? Its just all rather confusing. Someone is suppose to phone me back on Monday to go through things with me.

Not sure if I have been any help?

Lisa

I presume it refers to statutory sick pay, but other than that I have no idea.
When I worked in a benefits office, so many forms were not used any more, and different people had different ideas on how to apply for things. Very confusing.

SSP1 is given to you by employer when coming to end of ssp statutory sick payments, then Job centre plus send you a SC1 asking why you cannot work and a med4 (sicknote, then you are asked for a PCA form (personal capabilty assessment), and eventually an IB50 - which is all about what you can do and cannot with regards to getting out of a chair/climbing stairs/writing etc. You are then sent for a medical organised by JC+. You can actually ask for an exempton from this on the form IB50 because of your progressive impairment of cardio and respiratory function and get the benefit. However if your NI contribution for previous 2 full tax years are incomplete you will only get NIcredits, which is towards your retirement pension.
Is that any help xxx
