Amikacin Nebulisers

Hi there, some advice needed here

I'm on Amikacin via a Nebuliser twice a day. Originally i found it ok to tolerate, but recently i have started using different nebuliser mouthpieces and have started being sick with each dose. My nebuliser mouthpieces have been:

Iso-Neb Filtered mouth pieces - These are too expensive to continue with, but i'm not sick quite so often when using them.

Pari Filter Kits - I can feel the drug irritating the back of my throat, eventualy i cough really hard and throw up any food i've just eaten.

Pari E-Flow - I don't tend to feel the drug irritating my throat but still vomit after dose. (i've only just started using this so it may take some getting used to shorter dose times)

I've tried taking metoclopramide before each dose but i still vomit small amounts of food.

Is anyone else on Amikacin nebulisers?

If so what do you find is the best way to nebulise this drug?

Any advice would be appreciated

Have you tried diluting the amikacin down with different amounts of saline to see if that helps you?

I use the eflow which fires the drug out super fast but in too concentrated form for me - so I dilute it a little more.

I dilute the 2ml of Amikacin with a further 2ml of Saline. Still no luck...

I'm going to have another go with 3ml of saline, making it 5ml in total which is the maximum amount of liquid for an E-Flow. Will have to see how that goes.

Or split the dose so that there is maybe 1ml of amikacin to 3 or 4 of saline and then repeat a bit later. That's what I do but it does mean more nebbing but the upside is less gagging.
