promixin before or after DNase?

The Ineb rep has just been round helping Tom sort out his Ineb (cleaning etc). Whilst talking we discussed how DNase works. He then said Tom should take DNase bofore Promixin. He has been doing it the other way round for years and I am sure that is what we were told. He rang a patient at Brompton who takes DNase first.

Dnase should be given first to loosen secretions. Then wait half an hour and do physio, paripep trainer etc. Then give promixin so it can get down to the bottom of the hopefully cleared out lungs!! Maggie

Yes, we've always followed those guidelines for any antibiotics - Dnase first, then physio (followed by lots of lovely coughing!!) then antibiotics.

Personally, having all day to do such things, I prefer to take DNAse late in the afternoon, so it has hours to work on my mucous. I then take salbutamol and physio, and finally take colomycin.

DNAse doesn't work if there is ny colomycin (promixin) or other antibiotic still in the chest, so you want to leave several hours after taking the antibiotic before taking the DNAse. You then need to leave time for the DNAse to work before doing any physio. That is why it is best to take the DNAse, wait, do physio, and only then take any antibiotic.
